On shift

One ED practice

This is the use DocuRounds can describe without inventing a study. Dr. Randell Chapman uses it on his own shifts. A timed before-and-after is not published.

Who

Dr. Randell Chapman, an emergency physician, on his own shifts.

What he signs

A field-free note. No brackets. No placeholders left for him to delete.

MDM

Medical decision-making language is written from the encounter he provided, including data he reviewed and the risk he considered.

Critical care

99291 and 99292 time is added when the encounter supports it. Separately billable procedures pause the clock.

Procedures

A procedure note is triggered by his input. It does not appear because another service did the procedure.

What stays out

Hallway talk, phone calls about another bed, shift-change handoffs, and side comments are discarded.

What this page does not claim

There is no published count of charts generated, and no published median for minutes saved. Those numbers are not on this site because they have not been measured for publication.

Physicians can create an account. The first 20 charts are free. Groups can write through the contact page.
