A loud ED is full of speech that is not the chart. Phone calls about another bed, hallway talk about the next admission, shift-change handoffs, and side comments are discarded. The note stays on the patient in front of you.

The same rule covers the rest of the draft. If you did not provide a procedure, a score, or a finding, it does not get written in to fill a template. Omission is preferable to fabrication.
